Information for Professionals
Advancing Minds staff understand that the best way to help the
children we treat is to work closely will all of the adults in
their lives, including parents, teachers, pediatricians, therapists,
psychiatrists, and many others. With parent consent, we are interested
in learning how you work with the children in your care and
what questions you may have for us as evaluators or therapists. Please
do not hesitate to call us if you have any questions.
Referral Process
Because many families find Advancing Minds through another professional,
we work with you in whatever referral process is most convenient for you. Some
methods of referring clients include:
- Phone at (408) 294-9903.
- Advancing Minds Referral Form (requires Adobe Reader).
This form serves as a structured way to discuss referrals with families, and to quickly and efficiently connect families with our clinic. Some referring professionals have found that this form helps streamline the referral process so clients will feel comfortable meeting and working with us. Clients may also indicate if they prefer to contact us or if they wish that we contact them. After completing the form, please fax it to
(408) 692-9988.
- Other referral forms. If you wish to use your own referral form, you may fax it to (408) 692-9988.
- Client contact. If the client wishes to contact us directly instead of having their information sent in advance, you may give them our contact information or brochure (requires Adobe Reader) or refer them to our website. If you need copies of our brochure or business cards with contact information, please call us at 294-9903 and we will send as many as you need.
Ongoing Communication
Advancing minds staff work in close communication with referring
professionals, during and after an assessment. After obtaining consent
from parents, we will call you and either ask for a copy or summary of the
child's records, or discuss your work with the child on the
phone. This will help us better tailor our recommendations to your
setting and gain a more in-depth picture of the child's progress over
time. We also set aside time to explain assessment results when possible
to referring professionals. In most cases, parents will ask that a
copy of our report be sent to you. In some cases we are able to meet with you
and the family at your location to explain results in more
detail and answer any specific questions that you have. When this is not possible due to distance or scheduling conflicts, we are available by phone to answer any questions you may have about the results.
Please do not hesitate to contact us if you have any questions or
concerns, or would like brochures and business cards
to distribute to your clients. We can be reached by
phone at (408) 294-9903.
